Each person who participates in the Floating Classroom Program will need to fill out and return permission slips to the coordinator for the date they are attending or they will not be permitted to attend. Forms need to be sent to your coordinator/payment person prior to the date of the field trip.
If possible, please use the pdf versions of these files. They will
print nicer. pdf readers are available for free, click on the link
below if
you don't already have pdf on your computer. If for some reason you are
unable to use the pdf version, then print the html version on your
printer
and simply cut off the url, before you turn in the forms. If you have
more
than one student attending, you will need forms for each child, so
print
muliple copies.
The first form is a permission to use photographic images. You can
either grant or deny permission for the Floating School program to use
any photographs in their publicactions that were taken of you or your
children during the Floating School Program. Circle your choice on the
forms.

The second type of form that you will need to turn into your
coordinator prior to this trip is a general permission form. Because we
will be going out onto the water, we need emergency contact information
and permission to seek medical treatement for your child should an
emergency arise and
you are not available.
